Since, I am certified in massage, reiki, yoga and many different forms of energy healing - and a degree in psychotherapy I have included everything body - mind - and spiritually. This is to give a over - all view of what is needed from the client (and no surprises for me) - then I have the SOAP chart which is included on the intake form and when I have repeat customers I have a short form. That is the long form it has 10 pages - and yes, I give my clients fair warning that they will be filling this form out - before I touch them. Part Two: Could you please explain the meaning and reasoning as to why we use these forms - and the difference between 'spas' and ' private practitioners'. Thanks Valyncia PS - spas don't do long intake forms that pertain to the customer for the day. At the spa you fill out the short form - if any - then on the table you go.
